Outpatient competency restoration is expanding across US jurisdictions under pressure from forensic bed waits and least-restrictive-setting obligations, and this review maps the 19 programs identified while reporting that the evaluations supporting them remain "limited in size, scope, and methodological rigor" (Psychol Serv 2026), even as reported restoration rates track those of hospital and jail settings.
